Care Management provides visiting registered nurses to coordinate services between; clients, physicians, medical facilities, insurance providers, guardians/courts and or family representatives. It is very easy for a senior to get lost “in the system”. Working under the directions of our client, or designated family member, the visiting nurse helps medically frail clients through the health care system and provides advocacy to assure prompt medical response, in situations where seniors are in a “failure to thrive” situation. Services can be provided in long term care settings such as assisted living centers, nursing homes, adult family homes, or in private residences. This service can be packaged with other Elder Options services.
Nurses also provide; insurance physicals, long term care insurance advocacy, wellness checks, and site quality assurance visits for families who want to be sure their family member is getting the best of care.
Our RNs do not take the place of Medicare reimbursed nursing services. We do refer to home health care agencies if the service is paid for by Medicare.
